Our Transitional Care Programs offer hospitals, rehabilitation facilities, and other institutional settings reliability in knowing that our entire clinical team will work with patients and their caregivers to ensure that re-hospitalizations are avoided and improvement goals are met.
Our patients and their loved ones can also feel confident knowing that our nurses, therapists, and home health aides are known to deliver safe and compassionate care to help them recover after surgery or illness in the comfort of their own homes. We will develop a patient-centered Plan of Care and work closely with physicians to provide a plan which is specific to each individual that we care for.
