Because of our extensive network of resources in the community, our Life Care Transitions programs provides the assistance that you need to face some of the challenges of aging in place. We connect you with the resources needed to help you make the right decisions for yourself or your loved ones.
Life Care Transitions
Our goal is to provide you with the framework needed to make all important decisions related to aging in place. From understanding the programs and resources available to you, to transitioning out of the home into other living environments, we are here to assist you every step of the way.
ACCESS Florida Community Partner
Through a collaborative effort with the county, as an ACCESS Florida Community Partner, we can assist you with enrollment in several local and state funded programs that you may be eligible for.
Our team can complete a needs assessment to help determine your eligibility in numerous programs including:
- Florida Statewide Medicaid Managed Care Programs
- Veterans Programs
- Older Americans Act
- Agency for Persons with Disabilities
Clinical Care Management
Our in-house team of Clinical Managers can provide the assistance needed to communicate with hospitals, physicians, medical groups, and other organizations responsible for the coordination of your care. These services include:
- Ensuring medical supplies and safety devices are coordinated
- Ensuring prescriptions are ordered and delivered
- Ensuring important documentation is completed
Education and Advocacy
Our Community Relations Coordinators offer a wealth of knowledge and resources available to clients and their loved ones, including referrals of expert services such as:
- Elder Care Planning
- Legal and Financial Services
- End-of-life Planning
Transition to Independent, Assisted, or other Long-Term Care Living Facilities
